The survey will require 10 to 15 minutes to complete. All respondents will remain anonymous, and all answers are confidential. To protect your information, please do not enter your name, contact info, or other personally identifiable information in the online form. Your responses will be processed confidentially and only group data will be made publicly available.
Paper copies of the survey are also available at the library information desk.
